Application Forms

To study on any of YWAM UK&I's Discipleship Training Schools or Year Out courses you will need to fill out a number of forms. These are available below in PDF format. Please note that Operation Year has a different set of forms, also available below.

Simply choose whichever set of forms is relevant to the course you are interested in, then click on each of the links 1-7 or, if you want all 16 pages of forms in one go, on the Application Pack link 8. Provided your machine has Adobe Acrobat Reader 2.1 or later, the file will open. (If you find you need Acrobat Reader, go to adobe.com/acrobat to download a free copy.) You can then print the file as it is, fill them in and post them to the relevant YWAM centre.

We recommend that you read the Information and Financial Policy document (link 1 or pages 1-2 of the Application Pack) before beginning to fill in the forms.

Please note that you will need to complete all the forms in documents 2-7 or pages 3-16 of the Application Pack to be considered for a course with YWAM UK&I. We also recommend that you are fully aware of YWAM's financial policy as described in document 1 or on page 2 of the Application Pack.
